Since this is on school property all coaches and assistant coaches must be CORI’d with the school administration. The school department CORI lasts for 3 years and you may already be CORI’d with your child’s school so you should be on record. Please let your appropriate Director know. If not you would need to complete this form and have it authorised by someone at the School Administration on Harpin Ave.

We also are required to have you submit a CORI with Mass Youth Soccer but that can be done on-line.
